Amie Mouneimne may modify, amend, supplement and replace these Terms and Conditions at any time without providing you with advance notice. Payments + Services
All our services accept the following payment methods: payments on-line via our secured payment server Stripe via credit card, personal checks, e-transfers, and cash.
As we are registered in British Colombia, Canada, all our services are in CAD. Our on-line payments use the most advantageous conversion rate possible. We use Stripe as our payment platform. If you are paying in USD, your bill will automatically reflect the conversion rate of CAD into USD. Rates of USD to CAD change daily – if you want to know the daily rate, please check on-line.
Invoices will be provided by email. Special invoices and/or forms for insurance reimbursement purposes will be provided upon demand.
Signature Program – 3 Months
Please note that this is a minimum 3-month engagement with two different payment option plans. The Signature Program is valid for 3 months. If you cancel an appointment, you will need to book another appointment within this time limit. For rare circumstances, we may decide to extend your 3-month period. However, the 3 monthly payments will still be charged over 3 consecutive months.
Option 1 – 3 payments over 3 months. 1st payment due before the first consultation. By committing to this plan, you are committing to the 3 payments. No cancellations are permitted after the first consultation has been made. You will be able to set-up a recurring payment that is on-line via our secured payment server, use e-transfer or you can draw 3 personal checks which will be cashed over 3 consecutive months.
Option 2 – 1 payment of the total amount. You will receive a 5% discount for paying the total amount in one payment. This payment is due before the first consultation. No reimbursements will be made once you have made your payment. Your payment choices: credit card, e-transfer, check, or cash.
Signature Program – 6 Months
Please note that this is a minimum 6-month engagement with two different payment option plans. The Signature Program is valid for 6 months. If you cancel an appointment, you will need to book another appointment within this time limit. For rare circumstances, we may decide to extend your 6-month period. However, the 3 monthly payments will still be charged over 3 consecutive months.
Option 1 – 3 payments over 3 months. 1st payment due before the first consultation. By committing to this plan, you are committing to the 3 payments. No cancellations are permitted after the first consultation has been made. Option 2 – 1 payment of the total amount. You will receive a 20% discount for paying the total amount in one payment. This payment is due before the first consultation. No reimbursements will be made once you have made your payment. Your payment choices: credit card, e-transfer, check, or cash.
VIP Monthly Tune-Up
Please note this is a recurrent monthly on-line payment. It is open to clients who have completed our Signature Start-Up Program. You may join with the flexibility to cancel at any time.
If you wish to join, we will send you an invoice per your request to join.
There is no time limit to this program. Join and cancel any time. It includes: 1hr monthly consultations, email and phone call support, and updated wellness plans.
A la carte Tune-Up – 2-part Consultation
This is a 2-part consultation. Full payment is due before the first consultation. It is open to you if you have completed the Signature Start-Up Program. The first consultation is 1 hour and we will address how you have been doing since the Signature Start-Up Program and new concerns or goals you have. The second consultation is 30 minutes and includes a presentation and Q+A of a full Wellness Plan. The second consultation needs to be scheduled within 1-2 weeks of the first consultation for it to be the most helpful and up- to-date.
One hour Counselling
These are one-hour consultations and do not include meal or wellness plans. Full payment must be received before the consultation.
Cancellations + Reimbursements
If you are unable to attend one of your consultations, you may cancel within a minimum of 24 hours or more of your scheduled appointment. For cancellations, please cancel on-line and send us an SMS text message within 24 hours or more notice. You may rebook your consultation. If less than 24 hours notice is given, we reserve the right to count this as one of your consultations.
“No Shows” – If you do not show-up to one of your appointments and no notice is given, it may still count as one of your consultations.
Once you have made a payment and had a first consultation, no reimbursements (partial nor complete) will be made.
Full reimbursement will be made only if you have made a payment and decide to cancel 24 hours or more before your first consultation.
Confidentiality + Privacy Agreement
Please note this program is personalized for you and is not intended to be shared with other people. We will not share any of your personal information. Any documents we collect are confidential and are not shared with third parties. On some occasions, we might ask your permission to share some of your information with other health professionals to assist you better. However, we would first get your consent and we would not share your name or other personal information such as addresses, phone numbers, etc.
Your consultations are for you only and guests are not invited to our consultations unless other arrangements have been made with Amie Mouneimne.
Applicable Law
By visiting this website, you agree that the laws of the province of British Colombia, Canada, without regard to principles of conflict laws, will govern these terms and conditions, or any dispute of any sort that might come between Amie Mouneimne and you, or its business partners and associates.
Disputes
Any dispute related in any way to your visit to this website or to products or services you purchase from us shall be arbitrated by provincial (British Colombia) or federal (Canada) court and you consent to exclusive jurisdiction and venue of such courts.
Indemnification
You agree to indemnify Amie Mouneimne and its affiliates and hold Amie Mouneimne harmless against legal claims and demands that may arise from your use or misuse of our services. We reserve the right to select our own legal counsel.
Limitation of Liability
Amie Mouneimne is not liable for any damages that may occur to you as a result of your misuse of our website or services. Amie Mouneimne reserves the right to edit, modify, and change this Agreement any time.
Entire Agreement
These Terms and Conditions and any other legal notices, policies and guidelines of Amie Mouneimne linked to these Terms and Conditions or contained on this Site constitute the entire agreement between you and Amie Mouneimne relating to your use of this Site and supersede any prior understandings or agreements (whether oral or written), claims, representations, and understandings of the parties regarding such subject matter. This Agreement may not be amended or modified except by Amie Mouneimne. If for any reason a court of competent jurisdiction finds any provision or portion of these Terms and Conditions to be unenforceable, that portion or provision shall be enforced to the maximum extent permissible so as to effectuate the intent of the parties as reflected by that provision, and the remainder of these Terms and Conditions shall continue in full force and effect. Failure by Amie Mouneimne to enforce or exercise any provision of these Terms and Conditions shall not constitute a waiver of that right. Paragraph headings are for reference only.
